Sri Vijaya Puram, July 22: An Identification & Assessment Camp (Medical Assessment) for CwSN under Inclusive Education will be organized on July 29 & 30 and on Aug. 1 & 2 for the students from Anganwadi to Class XII under BPO South Andaman.

In this camp, the Medical Officer and ALIMCO team will assess children and recommend suitable aids and appliances under the ADIP-SSA scheme and also assess suspected students and recommend disability certificates.
Documents required for ADIP-SSA scheme are Copy of valid Disability Certificate or Joint Disability Certificate (format enclosed); Copy of UDID card/ Enrollment number of UDID; 02 nos. of passport size photographs; Copy of Income Certificate: Not exceeding Rs. 2,70,000 per annum (not older than 6 months) from Revenue Agencies or BPL Card or MGNREGA Card or Disability Pension Card or Certificate from MP/ Councilor/ Gram Pradhan (format enclosed) and failing which Notarized Affidavit; Copy of Aadhaar Card /School ID Card/Bank Passbook from a nationalized bank with photograph etc and Bonafide Certificate from the school. (Only for visual disability).
