Sri Vijaya Puram, Aug 03: The Identification and Assessment Camp for CwSNS scheduled on 04.08.2025 at Vivekananda Multipurpose hall, Diglipur from 9.00A.M. Onwards under BPO Diglipur Samagra Shiksha.
The purpose is to inform guardians, parents of children attending Anganwadi Centres, and the general public about the availability of disability assessment, certification, and distribution of aids/appliances, with a focus on children below 18 years of age. Your support will help reach the maximum number of beneficiaries
Documents required for ADIP-SSA scheme:
1. Copy of valid Disability Certificate or Joint disability Certificate
2. Copy of UDID card/ Enrollment number of UDID.
3. 02 nos. of passport size photographs
4. Copy of Income Certificate: Not exceeding Rs. 2,70,000 per annum (not older than 6 months) from Revenue Agencies or BPL Card or MGNREGA Card or Disability Pension Card or Certificate from M.P/ Councilor/ Gram Pradhan and failing which Notarized Affidavit.
5. Copy of Aadhaar Card /School ID Card/Bank Passbook from a nationalized bank with photographs etc.
Bonafide Certificate from the school. (Only for visual disability)
